Nominee Declaration Form
Made under Rule 11 of the Co-operative Societies Rules, 1998. Name up to five nominees and their shares, signed before two witnesses.

Member Loan Application Form
The six-part loan form covering filling instructions, applicant and loan details, guarantors, employer or group certification, appraisal and approval.

Before you submit
Five things that hold applications up
Almost every delayed application comes down to one of these.
- Loan applications must be made on the original form only — photocopies are not accepted.
- Attach a copy of your National ID and, where you are employed, a recent payslip.
- Members in non-formal employment attach business financial statements for the previous three months.
- Business loan applicants must submit a forecast cash flow covering at least six months.
- Guarantors must be members, and their signatures must be witnessed.
